DECLARATION OF OWNERSHIP AND ACCEPTANCE OF SPECIAL CONDITIONS FOR CRYPTO-ASSETS

Declaration of ownership

By signing this form, in writing or electronically, the Holder/Client hereby expressly declares the following:

  • The wallet addresses I provided for receiving crypto-assets from Dukascopy Bank ("Dukascopy") belongs to me and only to me. I hereby commit not to transfer it to anybody, as long as it is used in connection with my Dukascopy account ("the Account").
  • I am the sole beneficial owner of the crypto-assets I may send to Dukascopy.
  • I will send crypto-assets to Dukascopy only from wallets belonging to me and only to me.The Holder/Client undertakes to immediately inform Dukascopy Bank SA of any changes in the above information. It is a criminal offence to deliberately provide false information on this declaration of ownership (Art. 251 of the Swiss Penal Code, document forgery; penalty: imprisonment for up to five years or a fine).

Acceptance of special terms & conditions

By signing this form, in writing or electronically, the Holder/Client hereby further expressly declares the following:

  • I have read, understood and accepted all the White Papers, the risk brochure if any, limitations, procedures, fees and risks specific to the transactions in crypto-assets with Dukascopy, as published on Dukascopy's websites or contained in the account opening documents. I further accept that Dukascopy may restrict my use/my access to the Account if it deems it necessary to manage its risks and/or should any present declaration be incorrect or false.
  • I understand and accept that crypto-assets may be very volatile and lose their value.
  • I confirm that I will never deposit either directly or indirectly crypto-assets connected to an offense and that I will not use crypto-assets provided to me by Dukascopy to circumvent embargos/sanctions or in the context of illicit activities.
  • I had the opportunity to ask all my questions to Dukascopy and all of them were answered to my satisfaction. I would not sign the present form if I would have any concern or open questions about the Account in general, its functioning, fees and the quarantine procedure.
  • I hereby expressly agree to indemnify and hold Dukascopy, its employees and agents harmless from any and all liability, damage or costs whatsoever should any above declaration proves to be incorrect or false.
